Items Needed for Loan Funding
- Proof of income – ALL borrowers (Recent paystub or bank statement showing direct deposit. Self-employment income requires two years tax returns.)
- Drivers license copy – ALL borrowers (If address listed is different from current address, provide copy of a recent utility bill or bank statement with current address.)
- Invoice from Dental Assistant School for payment remittance.
- New account card (Only required for new members).
